Intake Process for Sertoma Centre

Individuals interested in touring any of our Adult Day Program Sites or Residential Programs for Individuals with Developmental Disabilities can contact Lynn King, Consumer Support Services Coordinator via phone or email at (708) 730-6352 or lking@sertomacentre.org

Tours are available Monday through Friday between the hours of 9:00 AM and 3:00 PM.

If an individual/family is interested in being considered for admission into one of the programs they will need to forward referral information to us. If the individual is already funded via the Department of Human Services-Developmental Disabilities Division Medicaid Waiver program then they can request their ISSA provider (Suburban Access, Service Inc., or Community Service Options) to forward the referral information.

Once the information is received it is forwarded on to the specific departmental intake team for review. If the team feels they would like to consider the individual for services then a 1-2 trial day program visit may be scheduled.

For Residential programs individuals being considered would first come to the CILA home for a dinner visit, then proceed to an overnight and then weekend visit prior to an admission decision being made.

After the trial visit(s) if the team and individual/family are in agreement that the services can meet the individual’s needs then a start date for services will be established once funding is in place or the private pay agreement is arranged.

If the individual/family is not funded and they are interested in private pay services then they will need to submit their own referral packet for consideration.

The Mental Health Services Program provides individual and group counseling, educational groups and community based support to individuals with a primary diagnosis of mental illness. Individuals must typically be enrolled in Medicaid and have a qualifying diagnosis. The program also offers community based supports as well as crisis linkage and referral.

Packets Submitted from Referring Agencies and/or family members seeking private pay services should include the following information if possible:

  • Introduction letter
  • Psychological Evaluation (current within 5 years)
  • Psychiatric Evaluation (within 1 year if applicable)
  • ICAP Evaluation (within 1 year)
  • Original Award letter for Medicaid Waiver funding from DHS OR Annual redetermination of eligibility from ISSA*
  • Copy of State ID, Social Security Card, and Medicaid Card
  • IEP/Individual Service Plan
  • Behavior Intervention Plan (if applicable)
  • Behavior Data (Last 6 months-if applicable)
  • Guardianship Papers (if applicable)
  • Recent Physical Exam (with in 1 year)
  • Current List of Medications
