Developmental Training (123rd St./ 127th St.)
Building Skills to Maximize Independence
Program Description
The Developmental Training Program supports adults with developmental disabilities through an emphasis on personal growth, productivity, and integration into the community and the workforce.
Additional Supports
- Transportation is available through PACE and private pay
- Services are provided both on- and off-site
- Community integration opportunities help generalize the skills learned
- Coordinated case management provided by Qualified Intellectual Disability Professionals (QIDP)
- Support from contractual Board Certified Behavior Analysts (BCBA) available. Services include behavioral assessment, development of a behavior support plan, and ongoing evaluation and supports
Program Highlights
- Volunteer opportunities available in the community
- Participation in community events and recreational activities
- Music and Art Therapy available
- Gardening program available through support of University of Illinois at Chicago
- Opportunities to transition to other services are available based on needs and preferences
- Available Monday through Friday, 8:30am-3:30pm
Admission Criteria
- To be eligible for Developmental Training, a person must:
- Be at least 18 years old
- Have a diagnosis of a developmental disability; other disabilities may be present
- Be able to adapt to a group setting within a reasonable timeframe
- Be willing to participate in the program and attend on scheduled days
- Be able to self-medicate if medications are taken during the day program hours
- Meet funding requirement, which is usually Medicaid, or have ability to pay privately

Home Based Support Services
The Help You Need at Home Starts with Home –Based
Program Description
Home Based Support Services (HBS) provides self-directed assistance (SDA), support planning and supports to adults with developmental disabilities to prevent or delay out of home residential services. Individuals and families choose from an array of services that include needs assessment for services, day programs, direct services provided by domestic or other employees, as well as a variety of therapy and other services, such as adaptive equipment. Home Based Support Services are tailored to meet individuals’ evolving needs and preferences for support, while maintaining their current residences.
Additional Supports
- Assist the individual and family in arranging for, directing and managing services
- Offers practical skills training to enable individuals and families to independently direct and manage Medicaid Waiver services
Program Highlights
- Serving individuals with Adult Home-Based Support (AHBS) Medicaid Waivers
- Services can be provided in the home of the individual or in the community
- Services to promote skill development and community linkage including the State appointed fiscal agent ACES$
- Coordinate with other programs in which individual/family is already involved
- Provide skills training to help individuals and families to recruit and hire Personal Support Workers (PSW) as well as provide information on effective communication
- Assist individuals and family with prior approval requests, such as adaptive equipment, home & vehicle modifications, assistive technology, and therapies
Admission Criteria
To be eligible for Home Based Support Services, a person must:
- Be at least 18 years of age or older
- Must be a resident of Illinois, living in Illinois
- Must be in the geographical service area
- Must have an intellectual disability; other disabilities may be present
- Be assessed as eligible for ICF/MR level of care
- Meet Medicaid eligibility criteria and have funding approved prior to starting services
- Agree to Rule 120 guidelines
- Agree to participate in the development and implementation of their person center plan

Residential Services
Community Living
Program Description
Residential Services provides small Community Integrated Living Arrangements (CILA) for adults diagnosed with an intellectual/developmental disability. Several different types of living arrangements are available including Intermittent CILA services, generally provided in an apartment setting, and CILA, a group home model with staff on site 24 hours a day. Support services are coordinated that reflect the individual’s needs and preferences.
Additional Supports
- Coordination of community resources and supports to meet individual needs such as medical, dental, educational, psychological, social, etc.
- Coordinated case management provided by Qualified Intellectual Disability Professionals (QIDP)
- Support from contractual Board Certified Behavior Analysts (BCBA) available. Services include behavioral assessment, development of a behavior support plan and ongoing evaluation and supports
Program Highlights
- Community activities and community integration
- Opportunities and support to develop and maintain natural supports, relationships
- Opportunities for skill development and growth in areas of self-advocacy, economic self-sufficiency, socialization, use of community resources, daily living and independent living skills, self-administering medications
Admission Criteria
To be eligible for Residential Services, a person must:
- Be at least 18 years of age or older
- Have a diagnosis of a developmental disability
- Be in need of a supervised living arrangement
- Need an array of services
- Be able to give informed consent to participate in CILA themselves or through a guardian
- Agree to participate in the development and implementation of their person-centered plan
- Have Illinois Department of Human Services (DHS) approved funding

24 Hour CILAs: Alsip, Midlothian, Garden Homes, Oak Forest, Homewood, Flossmoor and Olympia Fields
Intermittent CILA: South Chicago and South Suburbs

Community Employment Services
Program Description
Community Employment Services provides the supports necessary for individuals to achieve successful employment in the community.
Services include:
- Individualized service design and planning
- Pre-employment assessment
- Job development
- Job coaching, if needed
- Ongoing supports.
Additional Supports
Community job trial sites help determine interest and ability.
Program Highlights
- 24 people with disabilities obtained community employment in the second half of 2018
- Employment has been secured in Chicago and the surrounding suburbs
- Some current employers: Steamatics, Ed Miniat, Salina’s, Christ Hospital, Double Tree,
- Mariano’s Panera Bread, Jewel-Osco and Culver’s
Admission Criteria
To be eligible for Community Employment Services, a person must:
- Demonstrate interest and potential to work in the community
- Have a reliable, accessible method of transportation available
- Be willing to fully participate in the services needed to secure employment
- Be willing to secure/provide the appropriate funding (Department of Rehabilitation Services (DRS), Medicaid Waiver, private pay)
