Program Description
Bridge services focus on people with severe and profound developmental disabilities and seniors who need a greater level of support. The program provides support in the areas of daily living, recreational and social skills by providing active engagement in meaningful daily activities.
Additional Supports
- Transportation is available through PACE or private pay
- Community outings help generalize the skills learned
- Music and Art Therapy available
- Coordinated case management provided by Qualified Intellectual Disability Professionals (QIDP)
- Support from contractual Board Certified Behavior Analysts (BCBA) available. Services include behavioral assessment, development of a behavior support plan and ongoing evaluation and supports

- Meaningful daily activities such as cooking, music, arts & crafts
- Frequent community participation in events and recreational activities
- Music and Art Therapy available
- Gardening program available through support of University of Illinois at Chicago
- Community access through participation at events and recreational activities
- Focus is on assisting with personal care needs and encouraging relationship building, maintaining and enhancing skill level
- Available Monday through Friday, 8:30am-3:30pm
Admission Criteria
To be eligible for the Bridge Program, a person must:
- Be at least 18 years old
- Have a diagnosis of a developmental disability; other disabilities may be present
- Be able to adapt to a group setting within a reasonable timeframe
- Be willing to participate in the program and attend on scheduled days
- Be able to have medical needs met by The Bridge Program
- Be able to self-medicate if medications are taken during the day program hours
- Meet funding requirement, which is usually Medicaid, or have ability to pay privately
