Residential Case Management

Sertoma provides residential case management for people seeking either to transition into and/or maintain community-based housing. Services include mental health counseling, crisis intervention, psychosocial rehabilitation groups, independent life skills training, community integration, substance-use treatment, and medication management training. The goal of residential case management is to ensure that people are able to remain in community-based housing through support, advocacy and care coordination services.

Permanent Supportive Housing

Sertoma is the property manager and owner of a 16-unit community apartment building for individuals with a disability. We primarily serve individuals with mental illness who require support services to live in an unsupervised apartment setting. Tenants have access to our Community Mental Health and Counseling services, or can be referred to another provider for needed support.

Do you have an apartment for me? Can you help me find an apartment?

Sertoma Centre does have a Permanent Support Housing building. However, we currently do not have any open units, and our waitlist has been closed by the Housing Authority of Cook County. Please frequently check the below websites for open waitlists to all potential housing opportunities.

How do I get a voucher? How do I get a subsidy?

In order to get a housing voucher or subsidy, you have to apply through your local agency (Housing Authority of Cook County or HACC, IHDA, CHA). Then it is determined if you qualify or not based on your past year’s income. We also suggest that you initiate or engage in support services so that you can receive support in navigating the application process.

Can you help me find housing for my [family member, friend, etc.]?

Sertoma Centre believes in the Housing First Model and is dedicated to the belief that everyone should have stable housing. However, we recognize that sometimes different levels of support are necessary to ensure successful independent living in the community. Please contact us so that we can discuss the different housing options that may be beneficial to your [family member, friend, etc].

Can you help me find housing for [family member, friend, etc.] if they live in a nursing facility?

Sertoma is part of the State of Illinois’ implementation of the Colbert Consent Decree. We provide individualized services as qualified members transition out into a community-based housing arrangement of their own choice.
