GOAL:
To provide an array of services which meet the needs of individuals with developmental disabilities who require a greater level of support in the areas of functional life skills, vocational skills and social skills while promoting personal growth, independence, community integration and the acquisition of new skills.
GENERAL DESCRIPTION:
The Personal and Community Services Program offers skills training designed to offer opportunities for continued learning with a focus on functional life skills development in a group environment. Services are coordinated through a person centered planning process, including formal assessment of needs and development of goals that reflect the individual’s personal needs and preferences. Instructional strategies focus on development of cognitive skills, interpersonal skills, communication skills and skills necessary to maximize independence in activities of daily living. Opportunities to develop academic skills in the areas of written language, money management and time orientation skills are available both through individual plan development and in a small group setting. The program also offers opportunities for community integration through outings and recreational activities. Individuals may also elect to participate in vocational activities on a part-time basis.
ENTRY CRITERIA:
- Must be eighteen (18) years of age or older.
- Must have a developmental disability; other disabilities may be present.
- Ability to adapt to a group setting after a reasonable time period.
- Willingness to participate in the program and attend on scheduled days.
- Program must be able to meet the individual’s medical needs.
- Meets funding requirements or has the ability to pay privately.
TRANSITION CRITERIA:
- Convene IDT team to discuss programming needs at annual ISP meeting or additional meetings as requested or needed.
- Refer to the Program Accessibility Chart for programs/services availability.
- Follow guidelines as outlined in inter-program transfer summary.
EXIT CRITERIA:
- The individual and/or guardian choose to terminate services.
- The individual’s physical disability or medical condition places the individual in danger.
- The individual’s behavior places the individual or others in serious danger.
- The agency can no longer meet the needs of the individual.
- Funding is terminated.
- Consumer is not benefitting from services.
- Service closure meeting is held, if applicable and follow-up is completed post discharge to address any additional needs.
SCOPE OF SERVICES:
Population served: Individuals over the age of 18 with developmental disabilities, physical disabilities and/or dual diagnosis.
Settings: Facility based and/or small group settings within facility.
Hours/Days of Service: Monday through Friday 8:30am-3:30pm
Frequency of Service: Part-time and full time status available within day program hours stated above.
Payer Source: DHS/DDD, private pay.
Fees: There are no fees aside from community outing costs in day programs or special events. Private pay rates have been established and are available as needed. Funding and rate structures are established and funded by state agencies as indicated.
