COMMUNITY SERVICES

Sertoma Centre, Inc. provides a number of person centered community residential programs.  These programs allow people with all levels and types of developmental disabilities to live as fully and independently as possible in their choice communities.  Through our innovative and progressive approach, we empower individuals with disabilities to live and participate in their communities as independently as possible.  We believe that people with developmental disabilities and their families know best when it comes to their needs, goals and planning for the future.

Intermittent CILA

Program for individuals who have some basic life skills and require specific supports for living on their own or with their families within the community. Individuals own, rent their own residence, or live in their family homes. This program has a support staff member that provides one on one tailored services for the individuals participating.
Staff is hired to assist people living in their home to learn how to manage daily activities. Community-based activities may include:

  • Assistance to effectively manage their homes, health, self-care, meal preparation
  • Access to community resources, community involvement, activities
  • Generic services, social relationships, working lives, shopping, banking, transportation, recreation and volunteering
  • Group learning or social activities.

Individuals are supported to develop skills that assist them in achieving greater independence as well as their hopes and dreams.  Advocacy, educational workshops and partnership building are also important components of this program. For more information on Intermittent C.I.L.A., click here

Home Based Support Services

Allows individuals and their guardians to select support services required in order to live successfully at home and to progress toward future goals, hopes, and dreams they may have.  Services and providers are chosen by the individual and guardian with professional and experienced assistance from the Home Based Support Service Facilitator.  As an added incentive the Home Based Service Facilitator at Sertoma Centre, Inc provides:

  • Advocacy
  • Educational workshops
  • Wellness classes
  • Partnership building

Which are all important components of our Home Based Program. Home-Based Support Services Program funds can be used for a variety of services based on individual needs including:

  • Personal Support
  • Developmental Training
  • Supported Employment
  • Adult Day Care
  • Regular Work
  • Nursing
  • Occupational Therapy
  • Physical Therapy
  • Behavioral Intervention/Counseling
  • Speech Therapy
  • Communication Therapy
  • Home Modifications

Adult Foster Care

Adult Foster Care Program provides 24 hours of support and services in a community home for adults with developmental disabilities.  This program focuses on community integration and choice while understanding the specialized and often complex needs of the individuals we serve.  This program environment provides a family atmosphere in which the adult served is a member of the family unit.

Foster families provide assistance with everyday activities like grooming and eating, as well as leisure activities. They also provide access to services which help individuals maintain or increase their levels of independence.
The individual homes are approved and monitored by the agency and must comply with regulatory or agency requirements.

Specialized Services

Specialized Services is a Bogard Service Coordination system that works with individuals who are class members of a lawsuit that was settled between the State of Illinois and the ARC of Illinois and eligible class members.  Eligible individuals can receive a variety of services and are provided service coordination and advocacy to ensure they are receiving the services they want and need.  Services are provided regardless of the living arrangement or need for Specialized Services.  The services provided are designed to help the individual achieve the goals of self-determination and independence.  Services Include:

  • Coordination of services
  • Development of single comprehensive service plan
  • Assurance of service plan implementation
  • Advocacy.
